Qualification Categories for Physicians: Description and Requirements for Obtaining Them
It is the duty of every practicing physician to improve their knowledge and skills. Certification is considered one of the methods of training, with its own requirements and characteristics, based on the results of which specialists are assigned corresponding categories. Each category of physicians occupies a specific level in the hierarchy of the medical field.

Purpose and Objectives
Participation of healthcare professionals in accreditation is voluntary. The process evaluates the specialist’s personal competence, level of knowledge, practical skills, suitability for the position, and professionalism.
Of particular interest is the accreditation of doctors by category.
- It is known for enabling one to attain a higher position within the healthcare organization and attract the attention of management. Often the category of healthcare professionals is indicated on a plaque at the entrance to their office;
- In some cases, a higher rank may reduce the moral or physical responsibility toward the patient’s parent. For example, it is difficult to imagine what would happen if such a highly qualified specialist were unable to resolve the issue, while an experienced physician were available;
- Financial aspects. An increase in the number of levels in a physician’s medical category and medical hierarchy may increase the base salary supplement.
Types of Certifications
The law distinguishes several types of certification procedures.
- Awarding the title of “Specialist” after assessing theoretical and practical skills;
- Medical qualification category (acquisition);
- Confirmation of category.
Determining the level of knowledge for the award of “Specialist” is an important step prior to a doctor’s appointment. It is conducted by a special commission of the postgraduate institution. The following candidates are considered:
- Internship, residency, or postgraduate studies, if there is no “Specialist” diploma after the internship;
- Those who have not worked in a narrow specialization for more than three years;
- Those who have not undergone timely accreditation to obtain the qualification;
- Individuals who were denied the opportunity to obtain a second category for objective reasons.
Every physician is eligible to receive categories in multiple specialties simultaneously, provided they are related. The main requirement is having the necessary professional experience in the specialty. The category for general practitioners is an exception.
Basic Rules and Requirements
There are second, first, and highest categories of doctors. The requirements apply to all categories, but there are exceptions. The requirements are listed in the table.

Validity Periods
A physician’s accreditation category is valid for five years following the signing of the admission order. After this period expires, the specialist must either renew their certification or obtain a different category.
Under the old procedures, there were certain circumstances that fell under the category of social benefits and allowed for the extension of the current period of eligibility for benefits. These included the following:
- Pregnancy and caring for a child under the age of three;
- One month after dismissal due to downsizing;
- Business trips;
- Conditions of temporary disability.
Currently, these benefits are no longer valid. The Accreditation Commission may decide to extend the term of office of the chief physician of a medical institution. If the physician refuses to rely on the commission, the category is automatically revoked five years after it was granted.
Documents
Four months prior to recertification, the specialist must submit an application to confirm or obtain a higher qualification. Passport details, existing categories, and the date they were obtained, with the individual’s signature.
Accreditation forms and reports on work performed in recent years are also completed by the chief physician of the medical facility and the human resources department (the accredited representative). Copies of documents related to education, employment records, and current professional qualifications are also submitted to the Commission.
Certification Report
First and foremost, it contains information about the nature of the physician and the medical institution where they hold their position. It describes the characteristics of the department, its equipment and staffing structure, and the department’s performance indicators in the form of statistical data.
The main section consists of the following points:
- General characteristics considered by the department;
- Capability to perform diagnostic measurements;
- Medical work with specified outcomes for specific diseases;
- Fatalities over the past three years and their analysis;
- Implementation of innovations.
The report’s conclusions include a summary of the results, highlighting potential issues, examples of solutions, and possible improvements. If any published materials are available, a copy is attached. A list of references used and researched in recent years is provided.
Points for Advancement
Each expert earns points for participating in decision-making related to their area of expertise. They are tasked with attending events such as international conferences, lectures for colleagues or healthcare professionals, online training programs leading to a certificate of completion, training courses, etc.
In addition, points are awarded for the following achievements:
- Publishing textbooks, manuals, and monographs;
- Publishing articles;
- Obtaining patents for inventions;
- Presenting papers at symposia;
- Institutional and media appearances;
- Receiving academic titles;
- Defending a dissertation;
- Awards from government agencies.
Composition of the Commission
It consists of a committee, which operates between meetings, and a specialized expert group (exams, tests) that carries out the direct accreditation of experts. Both the committee and the expert group consist of individuals holding the following positions
- Chair, who oversees the work among committee members and assigns responsibilities;
- Vice Chair, who performs all the functions of the Chair in his or her absence;
- Secretary, who registers incoming documents, prepares materials for the committee’s work, and reviews decisions;
- The Deputy Secretary acts as a substitute for the Secretary and performs his or her duties in his or her absence.
Each professional group comprises specialists in the relevant field. For example, the category of dentists and the process for obtaining or renewing their credentials must apply to the groups of periodontists, orthodontists, pediatric dentists, and general dentists.
Meeting Procedure
Accreditation is processed within three months of the Commission receiving the specialist’s data. If the data does not meet the latest requirements, the document is rejected (within two weeks of receipt). The Commission secretary agrees to chair the expert group for the relevant specialty on the day of the examination.
Members of the expert group review the certification work of physicians in this category and complete their evaluations, indicating the following information:
- The specialist’s level of practical skills;
- Participation in social projects related to the medical sector;
- Availability of publicly accessible materials;
- The certifier’s self-education;
- The alignment of knowledge and skills with the declared physician category.
The review must take place within two weeks of receiving the report. The outcome of the review serves as an indicator of the potential certification result. The secretary notifies the candidate of the date of the appointment, which includes an interview and a test. To pass the test, the candidate must answer at least 70% of the questions correctly. The interview consists of the certifier being asked questions based on theory and practice. Their knowledge must meet the required qualifications.
The session is documented in a protocol signed by the members and the chair of the expert panel. The final decision is recorded in the qualification sheet. Specialists are eligible to retake the test only after one year. Within seven days, the candidate receives a letter confirming the promotion, demotion, or denial of the category.
Extreme measures
The management of a medical institution may submit a request to the Commission for the early revocation of a certificate or a promotion of a physician. In this case, documentation justifying the decision is submitted. The committee reviews the matter in the presence of an expert. Absence without a valid reason allows for a decision to be made in their absence.
Appeal
Within one month of the decision being made, a physician or medical facility may file an appeal. To do so, they must submit a written statement outlining the grounds for the disagreement and send it to the Ministry of Health Committee.
